To ensure the highest standards of clinical accuracy and patient safety, we are updating our Guidelines for Answering Medication Math Questions. Effective immediately, half credit will no longer be awarded for answers that are incorrect due to rounding. In nursing practice, even small calculation errors can have significant consequences for patient care. Allowing partial credit for incorrect answers may unintentionally reinforce unsafe habits. This change is intended to promote precision, accountability, and the development of sound clinical judgment consistent with professional nursing standards.

The updated guideline clarifies rounding rules for conversions.

 

Guidelines for Answering Medication Math Questions

 

NOTE: Full credit will only be awarded if all work is shown and the problem is answered completely and correctly. Partial credit will not be given.

 

1.      Leading Zero Required; No Trailing Zero:

·         Always use a leading zero for decimal values less than one

o   Example: (e.g., 0.8 mL, NOT .80 mL)

 

2.      Unusual or Unsafe Dosages:

·         If a question requires administering an unusually large or small dose (e.g., 1/8 of a pill or an excessive number of tablets), students should indicate that they would question the dose and/or contact the pharmacy.

 

3.      Syringe Markings and Rounding Rules:

·         Syringes will be marked, if requested

·         For volumes greater than 1 milliliter (mL), round to the nearest tenth (one decimal place).

o   Example: 1.28 → 1.3; 1.24 → 1.2

·         For volumes less than 1 milliliter (mL), round to the nearest hundredth (two decimal places).

o   Example: 0.654 → 0.65; 0.135 → 0.14

 

4.      Syringe Compatibility:

·         If the syringe provided is not appropriately calibrated for the ordered dose, the correct answer is to obtain a properly calibrated syringe.

 

5.      IV Gravity Drip Rates:

·         Drip rates must be rounded to the nearest whole number.

o   Example: Calculator result of 20.8 → 21 drops per minute

 

  

When a conversions results in a decimal, it should be round to the hundredth place.
